Canadian Vet Clinic uses this form to collect enquiries about care packages for households with five or more pets.
A valid phone number is automatically stored when you enter it, even if you do not submit the rest of the form. Clear the phone field during the same visit to remove your unfinished enquiry and wait for the removal confirmation.
An unfinished form is shown to the clinic as a saved number with follow-up permission unconfirmed. Submitting “Request my tailored package” asks the clinic to call or WhatsApp you about that enquiry. This does not subscribe you to unrelated marketing.
The clinic receives the phone number first. Your name, area, household pet details, preferences and notes are saved when you submit the completed form. Please do not include payment details, identification documents or information about your own health in the notes.
Responses are stored with the site’s hosting service and available through a restricted, signed-in clinic dashboard. The clinic can search them and export them for handling enquiries. A random session identifier is stored in your browser so edits update the same enquiry.
If you allow ad measurement, the form loads Meta Pixel 1045714654539992. It reports a page visit and a Lead event after a successful completed submission. Saving a number alone does not send a Lead event.
The integration does not send your typed phone number, name, area, pet details or notes to Meta. Meta can receive technical information such as your IP address, browser, page URL and cookie identifiers. A random event identifier helps avoid counting retries twice. Ad blockers or declining measurement can prevent Meta attribution without preventing your enquiry from reaching the clinic.

You can ask the clinic to stop following up or to correct or delete your enquiry by contacting Canadian Vet Clinic through its official website. Please identify the phone number used for the enquiry so the clinic can locate it. The clinic should retain enquiry details only for as long as needed to handle the enquiry and applicable record-keeping obligations.
